in NZ the health insurance has an excess that you pay up-front, and then the insurance company pays for the rest of the medical procedure after that, although I think they usually have maximums they pay for any single surgery, like $110,000 maximum for a heart surgery or something, but the average cost of such surgery is something like $50-60k so the maximum is quite generous
