This is from an old school game called QUEST FOR GLORY: Trial by Fire. I uploaded this vid because I couldn't find it already uploaded. This is a remake by ADG Interactive.
I just remember being creeped out yet simultaneously fascinated by this remarkable character and the music matched that perfectly:
