All of these. First time should've been "ok, something's up." Second, "Hmmm, maybe she's not really that into me." And third...well, it should've been pretty damn obvious. The girl has others in mind. I know it's hard, but it's for the best. If you've really considered being married to her, that really, really sucks. Marriage won't change anything about her with other people, it'll only change a) how much it hurts you when you finally do split, and b) how much stuff she gets. I know this sounds bad, but when this crap happens, you gotta think about what you want.
The only thing I want to add is this: What she did is a classic trap. Had it done to me. About 2 weeks after, found she was cheating again, found that the guy was a third guy, and that she had been fucking him for over a month. If they cheat three times, they're not worth the trouble, I don't care what you're getting from it.
If they cheat even once they're not worth the trouble. I hold that as a firm principle.

Okay yes, I see why you say this for obvious reasons. But if you set up a relationship with this as a standard, you're setting yourself up for trouble. They'll cheat and then never confess, and after realizing they are getting away with it will keep going. If they confess, its because they want to make things work between the two of you. Sure, make them work for your trust again, whatever. If it happens again, then its obvious they don't want things to work.
Most of the time women cheat, they aren't caught. They confess.

Reading your first post after you got back, I'd have said "stick with her in the short term and see how it goes". If things don't feel better, then cut it.

In any long-term relationship that is on the rocks, almost always both parties will be to blame at least in some aspects. Often one party could take more of the blame than the other, but it is very seldom that one party is completely faultless.
So I'm not sure what you should do at this point, don't really know enough about the situation. The fact that you are both living together, sharing expenses, makes things much more difficult to separate if you do break up, of course.
For god's sake, make sure you don't get her pregnant! In fact, I would be cautious about any contraception that you are using - she could purposefully fail to take the pill, or poke holes in your condoms (although you're probably not using them, I guess). Thankfully there are new long-term male contraceptives being worked on, where you get an injection and are sterile for several months afterwards, but I don't believe any of them are publicly available yet.

I question how sterile you are. Aren't injections for women right now between 33-66% effective? o.O

I'm sure that any male contraceptive injections would need to be at least as good as condoms before they were certified for use. And actually condoms aren't very good at birth control anyway. When used ideally, they are good, but real-world usage has much higher pregnancy rates. I'm sure you can google around to find out more about that, wikipedia probably has a good summary. Incidentally, reading the booklet that came with a packet of condoms, "family planning" eg natural methods had the lowest ideal failure rate of of 2% of the methods they listed, but the real-world rate was more like 30%.

Yeah I can see the weakness behind sticking to such a principle. I suppose the solution is to just not tell them about it until you have to.
