This shouldn't be involved. The 'town moron' is a highly strategic role, if you can think of it that way. The strategy involved is that the town doesn't want to kill him, and the Mafia killing him would be a waste (i.e. It's smarter to hit a special role). Therefore, he may be able to openly admit to being the doctor, and the Mafia may end up killing the town moron, or it might happen the other way around, the doctor admits his role because he's confident the Mafia will think he's the moron...
